연구 소개 · Brief Summary
염증성장질환(크론병, 궤양성대장염)은 장에서 만성적이고 재발성의 염증이 일어나는 난치성 면역매개 질환으로, 최근 국내에서도 발생률이 빠르게 증가하고 있습니다. 본 연구실은 이 질환의 발병에 핵심적으로 관여하는 장내 미생물(마이크로바이옴)에 주목하여, 기초 발견을 실제 치료제 개발로 잇는 중개 연구를 수행하고 있습니다.
연구의 중심에는 한국연구재단 「마이크로바이옴 기반 차세대 치료원천기술개발」 사업(2023–2027)으로 추진 중인 '염증성장질환 다중오믹스 기반 마이크로바이옴 치료제 개발' 과제가 있습니다. 서울대학교병원을 주관기관으로 울산대·POSTECH·전북대 및 산업체가 참여하는 다기관 컨소시엄이며, 단계평가에서 최우수 등급을 받아 2단계에 진입하였습니다.
본 과제에서는 국내 최대 규모의 염증성장질환 및 고위험군 코호트를 구축하고, 16S·shotgun 메타지놈, 진균체(ITS), 대사체, 단백체, 단일세포·공간전사체를 아우르는 다중오믹스 자원을 확보하였습니다. 이를 통해 질병의 아형(endotype)과 예후를 예측하는 모델을 정립하고, 진균체 불균형과 항-공생균 항체가 장벽 손상 및 면역 이상과 연관됨을 규명하였습니다.
특히 환자 분변을 이식한 인간화 무균 대장염 모델을 활용해 새로운 발병 기전을 밝히고, 29종 이상의 후보 균주를 스크리닝하여 항염증 효능을 가진 대표 파마바이오틱 균주(Clostridium leptum, Bifidobacterium longum)를 발굴·검증하였습니다. 나아가 궤양성 대장염 대변이식(FMT) 코호트를 통해 치료 반응 예측 데이터를 산출하고 있습니다.
궁극적으로 본 연구실은 마이크로바이옴과 인공지능 기반 멀티오믹스 분석을 결합하여 환자 맞춤형 정밀의료를 실현하고, 한국 최초의 염증성장질환 마이크로바이옴 신약 임상 진입을 목표로 연구를 이어가고 있습니다.
nflammatory bowel disease (IBD; Crohn's disease and ulcerative colitis) is an intractable, immune-mediated disorder characterized by chronic and relapsing inflammation of the gut, and its incidence has been rising rapidly in Korea. Our laboratory focuses on the gut microbiome as a central driver of this disease, pursuing translational research that connects fundamental discovery to the development of real therapeutics.
At the core of our program is the National Research Foundation project "Development of Microbiome-Based Next-Generation Therapeutics for IBD through Multi-Omics Integration" (2023–2027). Led by Seoul National University Hospital, this multi-institutional consortium includes the University of Ulsan, POSTECH, Jeonbuk National University, and industry partners. The project received the highest grade in its mid-term stage evaluation and has advanced into its second phase.
Through this work, we have established one of Korea's largest IBD and high-risk cohorts and assembled an integrated multi-omics resource spanning 16S and shotgun metagenomics, ITS mycobiome, metabolome, proteome, and single-cell and spatial transcriptomics. These data have allowed us to define IBD endotypes and prognosis-prediction models, and to demonstrate that mycobiome dysbiosis and anti-commensal antibodies are linked to epithelial barrier disruption and immune dysregulation.
Using humanized germ-free colitis models derived from patient fecal transplantation, we have uncovered novel disease mechanisms and screened more than 29 candidate strains, identifying and validating lead pharmabiotic strains (Clostridium leptum and Bifidobacterium longum) with anti-inflammatory efficacy. We are also generating treatment-response prediction data through a fecal microbiota transplantation (FMT) cohort in ulcerative colitis.
